To provide prevention activities to those cohort(s) to prevent or reduce negative health-related outcomes. The project cohort are: Individuals registered with a Luton GP with preselected diagnostic codes aged 1) 18-25 years and 2) 26+ years (based on existing SMI dashboard methodology). This is an area where spend has increased significantly in recent years without leading to significant and sustained improvements in outcomes for service users or their families/carers. The project will be designed to identify those ‘at risk’ of exacerbation more accurately by using more than clinical indicators. Although service users are the intended primary beneficiaries it is expected that families and carers will benefit from the project. By using data in these joined up ways, people/cohorts can be identified early and supported more proactively to prevent crises The business problem you need to solve 1) People on the SMI register regularly have crises. How can we identify early intervention for people on the SMI register using real time data held in local authority and health datasets to intervene/prevent? 2) Information of 'at risk' individuals is disjointed, sporadic in quality and captured in multiple systems that are not connected. This means care and service provision is also disjointed and reactive, rather than proactive. How can we be more proactive and preventative? 3) Can we identify and link these datatsets to identify key risk factor trends and highlight/flag areas of increased risk? 4) Health and social outcomes for this 'at risk' cohort has not improved at the rate of investment and public funding and budgets have no flex. How do we put in place a BAU outcome that delivers a cost neutral solution?
