Tender description

Oldham Council is seeking to commission a CQC registered external provider to deliver supported living services for people with complex mental health needs in their own home environment. The aim of the service is to move away from social and health care and focus on prevention and integration and a more person centred model of holistic care, offering a supported living style service within the community to support service users to become more independent. The contract is for a period of Three (3) years with an option to extend upto two (2) further years.
