Tender description
The Rapid Discharge and Transition block cars, or the Service, delivers domiciliary care to people in the county and allows the Council to meet the needs of Service Users being discharged from hospital quickly and effectively. The Service will be used to enable an individual to return home from hospital as soon as possible once they are medically fit, through the D2A process
