To provide Asbestos Management Surveys Lot 1: Carry out Asbestos management surveys of Queens Hospital, Burton-On-Trent & Sir Robert Peel Community Hospital, Tamworth. To provide complete reports to include • Photograph identifying the location. • General site and survey information • Survey results including asbestos type results. • Photographs of the locations of Asbestos material • Drawing plan indicating the location of Asbestos and the location of labels • Condition, recommended actions, • Analysis results • Asbestos register summary • Updated data spreadsheet to be completed as per the required format (ACM Import Data attached Appendix) to enable upload onto UHDB Trust Plant FM system. • Reports and ACM import data sheet are to be provide to the Trust within 21 days of completed surveys.
