Tender description

The 0 to 19/ 25 children’s and young people (CYP) wellbeing model of Stockton-on-Tees is part of a Public Health pathway of support for children and families from the antenatal period through to adolescents (or up to 25 years for individuals with special educational needs and disability, SEND). Stockton-on-Tees Public Health are seeking to engage the market to: • Inform future commissioning decisions • Understand the current market position • Describe the high-level service and seek initial insights from the market The services will include, but are not limited to: • 0-5 Health Visiting Services (including mandated contacts) • 5-19 Health Services for School aged Children (including a Tier 2 Healthy Weight Programme) • Screening including, NCMP, hearing and vision screening • Provide Public Health nursing input into the Children’s HUB (Front door to social care) Keywords: Children's Services, Public Health, 0-5 services, Health Visitor, School Nurse, Healthy Child
