Hospital Discharge Soft Market Testing Questionnaire

Liverpool City Council is conducting a soft market testing questionnaire exercise for services which support Hospital Discharge for patients on Pathway 0, and those needing support following presentation at A+E. The support provided will enable people to return home, increase confidence and retain independence which will aim to prevent future hospital admissions and a need for long term care and support. Support will include: • Help to travel home and support on arrival, including shopping, arranging delivery or collection of equipment and practical support to remain safe and well at home. • Providing a holistic approach to managing recovery, ensuring that social determinants of health are considered and managed. • Support to identify existing assets which a person can utilise to stay safe and well at home. • Information, advice and signposting to appropriate services including support to make referrals where necessary. • Where appropriate, liaise with professionals in relation to a person’s recovery. • Monitor referrals, engagement and outcomes effectively to demonstrate impact.
