SSD 23 05 Low Level Support Services

Tender description

Derbyshire County Council will be inviting tenders from providers for the provision of targeted, short term, low level, support services to deliver person centered, solutions focused, proactive preventative low-level support (not personal care). This service will work with the individual to identify any personal or environmental issues impacting on the person's ability to live as independently as possible in their own home. It will then work to support the person using a strengths-based approach to help them to help themselves. The contract length currently under consideration will be for 3 years, with the option to extend for 2 further period(s) of twelve months. DCC will be considering either a single countywide lot or splitting the county into two geographical areas. DCC will be open to bids from consortia with a lead provider or sole providers who can demonstrate how they would deliver the contract on a local District and Borough based footprint across Derbyshire. We are interested to hear the views of potential provider organisations on the delivery of low level support services.
