Development of an implementation plan for the Integration Index

Tender description

The Integration Index is a Long-Term Plan commitment to introduce a new nationally-coordinated data collection of the experience of integrated care, targeted at specific user groups and their carers. It will ask the different audiences a short set of meaningful questions in relation to care and support that span a number of different health and care services. As such, the Integration Index will be a source of important missing evidence about the performance of health systems across the whole of England, and is intended to support systems to identify issues and make improvements to care where this is needed.
