Tenderers must submit all requested information and documents including but not limited to the following: Completed (Signed and Dated): Sections 2, 3 and 4 of the *QC2 - Suitability Assessment Questionnaire The Following Appendices and Declarations Completed (Signed and Dated): *Appendix A *Appendix A1 *Appendix B1 *Appendix B3 *Appendix D1 *Appendix D *Appendix F And any other relevant supporting documentation/information as required. See ITT Section 5.2 Format of Tender Submissions and pg 36 of ITT Lot 237558: Envelope 2. Financial Envelope 2. Financial. Tenderers must submit all requested information and documents including but not limited to the following: Tender Price Submission Consisting of a Fully Completed:- Form of Tender and Schedule including Schedules A & B and relevant breakdown. and * Tenderers Quality Submission for each Criterion (or Sub-Criterion where used) as outlined and specified in the Particulars of this Instructions to Tenderers Document. See ITT Section 5.2 Format of Tender Submissions and page 36-37 of ITT document
