Wirral Council Weight Mangement Delivery Model

Tender description

The co-creation of a Wirral Council 12 week Weight Management intervention aimed at those with BMI 30+, BMI 28+ with co-morbidities, BMI 25+ Asian ethnicity and BMI 23+ Asian ethnicity with co-morbidities. Exclusions are pregnancy, severe to moderate frailty, eating disorders and unmanaged co-morbidities. This Weight Management project will contribute towards addressing health inequalities and improving general health and wellness across the Borough, with targeted groups being those from lower socio-economic areas and men. Participants will benefit from a hybrid approach which combines both nutritional education and guided lower level physical activity sessions. Consideration is given to breaking down barriers and encouraging a long-term behaviour change with the help of intervention from qualified professionals and peer support from those striving towards a similar goal. This is a client-centred approach and consideration is also given to those with health conditions and how we can adapt diet and physical activity to meet the needs of the individual participant on their journey.
