Dorset Accessible Homes Service including Sight and Hearing Impairment Service

<p>The service is being commissioned by the Adults and Housing Directorate of the Council. The new Dorset Accessible Home Service (DAHS) will enable vulnerable older and disabled individuals to maintain their independence, health, and well-being in their homes and community. The core service is funded by Dorset Council. The Disabled Facilities Grant funding comes from the Better Care Fund with some additional funding provided by Dorset Universities NHS Foundation trusts to fund minor adaptations to facilitate hospital discharges. The DAHS service will form the heart of our services for Dorset Council, putting Promoting Independence and Prevention to Support delivered in a strength-based holistic way. The Service Provider will build on the Innovation already started and work creatively to help us meet our strategic aims. The service will cover the Dorset Council area i.e. those individuals who pay their council tax to Dorset Council. The Individual groups will be those covered by Adult and Children&rsquo;s social care services future support needs resulting from (but not limited to): Old age Physical or sensory impairment Learning disability incl. epilepsy Mental health issues, including dementia Drug and alcohol issues Other vulnerabilities or impairments Children transitioning to Adult Services Children with Disabilities The service will also provide a range of interventions for sight and hearing loss, accessible across the county including: Advice and information Self and supported assessment Assistive technology and equipment &ndash; demonstration and provision Rehabilitation support &ndash; mobility, daily living, low vision support Links to leisure, training, education, employment and welfare support Links to community and peer support.</p> Keywords: Sensory Impairment, Rehabilitation support, Disabled Facility Grants, Accessible Homes, Maintain independence
