Tender description

Provision of community and accommodation-based housing related support to improve outcomes for vulnerable people who are homeless or at risk of homelessness. The Services will provide flexible and responsive support to vulnerable people who are in need of support because they are homeless or at risk of becoming homeless. This may include but not be limited to, people with substance misuse issues (including dependency and co-occurring conditions), mental health problems, physical disabilities or long-term physical health conditions, learning disabilities (where Service Users do not reach the threshold for adult social care) and those with a history of offending behaviours. The entire geography of Hampshire is not covered in this tender. Separate grant arrangements will be in place with Winchester City Council, New Forest district Council and Basingstoke and Deane Borough Council. Lot 1: Hart and Rushmoor Procurement of social inclusion support service to maximise the use of resources, avoid duplication, reduce bureaucracy and unnecessary process, and improve service access. The stage 1 model of support will provide accommodation with intensive support and staff available 24 hours a day, 365(6) days a year, to those who are homeless or imminently at risk of street homelessness Social inclusion is not a statutory service but HCC recognises care and support needs may be required as these service users cut across health, housing and social care therefore the aim is to have a recognised partnership approach for support of these service users. Lot 2: Fareham and Gosport Procurement of social inclusion support service to maximise the use of resources, avoid duplication, reduce bureaucracy and unnecessary process, and improve service access. The stage 1 model of support will provide accommodation with intensive support and staff available 24 hours a day, 365(6) days a year, to those who are homeless or imminently at risk of street homelessness Stage 2 accommodation-based support will provide less intensive support, helping individuals to realise their independence, working in partnership to support overall delivery of outcomes within the area where the accommodation is based. Generic community support services will provide crisis and prevention support to vulnerable people who are homeless or at risk of homelessness regardless of tenure, housing status or family structure. Social inclusion is not a statutory service but HCC recognises care and support needs may be required as these service users cut across health, housing and social care therefore the aim is to have a recognised partnership approach for support of these service users. Lot 3: Havant and East Hampshire Procurement of social inclusion support service to maximise the use of resources, avoid duplication, reduce bureaucracy and unnecessary process, and improve service access. Stage 2 accommodation-based support will provide less intensive support, helping individuals to realise their independence, working in partnership to support overall delivery of outcomes within the area where the accommodation is based.
