Continuing Healthcare Services in Nursing Homes - West Essex CCG

Tender description

This notice follows the publication of a Voluntary Ex Ante Transparency Notice published in the Find A Tender service reference number 2021/S 000-031536. The service is to deliver Continuing Healthcare Services to patients in Nursing Homes commissioned by West Essex Clinical Commissioning Group. The service supports people with long-term complex health needs who have been assessed, and qualify for, NHS funding. The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Support Tool. The service has developed the schedules within the NHS formal contract hat, when the contract is issued, will mitigate existing service risks by: - having a specific nursing home specification aligned to the 11 domains of the Decision Support Tool - An agreed rate for both nursing an dementia beds for the contract year - Evidence based requests for one to one support - Provider quality dashboard reporting - Expedites placements, delivering faster acute discharge and improving flow - Enables a strong partnership to be developed with the provider The contract is being awarded to several nursing homes within the West Essex area and will be for a period of 12 months with the option to extend for a further 12 months.
