Tender description

The Derbyshire Home from Hospital Support Service provides practical support for vulnerable people at the point of hospital discharge, or those in the community at risk of admission to hospital. The service supports people who may not be eligible for social care / NHS support but whose needs are likely to escalate without any intervention. The service offers initial practical support for up to 6 weeks to help the individual as they are recovering to remain living at home independently. This soft market testing exercise is to determine the capacity of the market to supply the service and the level of interest in any future procurement. The information provided through this exercise will not be used by the Council to assess bidders as part of the forthcoming tender.
