This Invitation to Quote is for enhanced and general medical services for service users residing in Shaw Healthcare (Barton Cottage) care home beds. The purpose is to secure a Service Provider to deliver enhanced and general medical services for service users residing in Shaw Healthcare (Barton Cottage). The Service is designed to provide an enhanced and general medical service for patients residing in Shaw Healthcare (Barton Cottage) care home beds. The Service will operate between the hours of 08:00 and 18:00 Monday to Friday excluding bank holidays. Outside of these hours the care Home Provider will contact the Out of Hours Service. This service specification supports: Discharge to access - allowing patients to leave hospital when clinically appropriate and supporting the significant challenges caused by delayed discharges and patient flow out of acute settings. The prevent of inappropriate admission to hospital for people who have a short-term episode of ill health, which does not require specialist medical intervention. Addresses unwarranted ambulance conveyance from the care homes by working collaboratively with care homes to monitor and identify appropriate care / interventions to support the patients to remain in the care home.
