Tender description
Objectives The objectives of the service are: The overarching objectives of the enhanced optometry services is to utilise the skills of primary care optometrists to triage, manage and prioritise patients presenting with eye conditions resulting in: Improved access and choice to services for patients, closer to home/more convenience Providing primary care ophthalmology services within the specified criteria Managing patients in primary care where appropriate Referring patients to the most appropriate service in line with the Clinical Guidelines, Protocols and Pathways agreed for Wakefield Improvement in patient experience and in clinical outcomes for patients Reduction in referrals into community and hospital services Contributing to developing and improving the programme year on year on the basis of evidence based returns including the annual report, data returns, external QA reports, audits and DH and NHS recommended service development advice current at the time Expected Outcomes The service will be available for all qualifying patients presenting or who are referred to the participating optician with an eye condition or complaint. The service will manage patients in primary care where suitable and in line with the agreed, Clinical Guidelines, Protocols and Pathways providing the appropriate level of care including onward referral for patients where appropriate. The service must be provided in line with national priorities, current and future recognised best practice guidance and standards in the field including: The College of Optometrists Local Optical Committee Support Unit Standard General Ophthalmic Services Contract The service provider shall ensure that the service is delivered in such a way that: Patient and carers have free access to appropriate evidence based information in a format that they understand.
