Tender description

NHS Hull Clinical Commissioning Group (CCG) is seeking to commission a broad range of Community Services through an open procurement process. The CCG wishes to establish a single Lead Provider contract for each of three distinct lots, also termed Care Groups. Care Groups are groups of services which the commissioner believes are naturally aligned and based around specific client needs/integrated pathways/service coherency. The three lots are: • Lot 1 - Integrated Sexual Health Service; • Lot 2 - Urgent Care; and • Lot 3 - Integrated Community Care Service. Lead Providers will be expected to demonstrate how they will deliver services in an environment of transforming health and social care provision as outlined in the Five Year Forward View, the commissioner’s strategy and Hull 2020 programme. Our aim is to achieve transformational change, including rapid response; care delivered at home as the default setting; seven day working, with 24/7 care available according to need. The service commencement date is 1 April 2016 for a duration of 5 years with the option to extend for a further 2 years. The Deadline for Submissions is 17:00 on Friday 29 May 2015.
