Tender description
Delivery of approximately 2000 NHS Health Check appointments/year using point of care testing and the PCT’s web- based delivery software within community settings. A health check assesses a person’s risk of developing CHD, diabetes, CKD or stroke, provides a CVD risk score and lifestyle service advice and service referral. Target Population: 40 to 74 year olds without existing disease invited for a health check
