NHS Calderdale CCG In-hrs Primary Medical Services to Intermediate Care Beds

Note on contract value: The requirement may be extended by a further 6 nursing beds, details of which will be submitted later (if this option is taken up, the total value of the contract will be £220,083). In Hours Provision of Primary Medical Services The purpose of the service is to provide in-hours primary medical services to adults in intermediate care beds who do not need to be in hospital, but who need a period of intensive rehabilitation before returning to their usual place of residence or agreed onward placement. NHS Calderdale CCG collectively commissions Nursing and Residential beds from local Care Homes from which Intermediate Care is delivered in conjunction with the SIT (Support and Independence Team). The purpose of the service is to provide in-hours primary medical services to patients in these beds. The aims and objectives for the service are to: • Provide high-quality person-centred assessment, care and treatment to patients who have been admitted to an Intermediate Care bed, referring on to other services where indicated • Encourage and support these patients to achieve and maintain an optimum level of independence and health • Contribute to the learning and knowledge of the staff providing care and support to the patients within the Care Home and through the SIT The service will be provided to the following beds in the following lots: Nursing beds • 15 beds in Home 1 (in the central Halifax area) • 8 beds in Home 2 (in the central Halifax area) • 7 beds in Home 3 (in the Brighouse area) Residential beds • 10 beds (due to the current procurement for the residential beds the locations to be notified on Bravo on the 11th July 2014). The cost of providing In-Hours Primary medical services for residential patients is £1300 per bed per annum. The cost of providing In-Hours Primary medical services for nursing patients is £3500 per bed per annum. The contract will have a contract term of 19 months from 1st September 2014 to the 31st March 2016.
