Mental Health Supported Living and Floating Support

Tender description

The Council is seeking tenders from experienced and qualified providers to provide care and support for Adults with Mental Health Needs. The Service is expected to comprise of three components for which the Council is seeking a single provider. The components are: Care and support to people living in a range of supported living schemes (the 'Schemes') located within the London Borough of Hillingdon; A Floating Support Service for people who have stepped down from the purpose-built supported living schemes, or who are living in a range of tenures, with the intention of preventing or at least managing an escalation of need that may otherwise lead to an admission into an acute mental health hospital; and An out of hours Telephone Support Service with the intention of preventing crises that are avoidable. The service will be delivered in three (3) supported living schemes, shared houses and within the community. The Council is seeking a provider with a CQC rating of Good or higher to deliver the service.
