Integrated Diabetes Service for North Lincolnshire CCG

Tender description

NHS North Lincolnshire Clinical Commissioning Group (CCG) is looking for expressions of interest for providers for their integrated diabetes service. This service will require consultant and diabetes specialist nurses to work on a seamless, integrated basis with primary care practitioners, which delivers a smooth pathway for patients and to give support and advise to the integrated teams across the five localities within North Lincolnshire. Delivery of a local, integrated, streamlined service which draws on professional and clinical input and experience from across existing primary and secondary care sources is fundamental to address local needs and deliver the desired outcomes: • Implementation of the new ‘super six’ integrated diabetes service • Improved patient choice due to wide range of available specialist treatment • Improved, robust education and skill development for primary care practitioners • Education and awareness programmes for ambulance and emergency crews – consideration of support for ambulance crews via a ‘hotline’ which guides and advises on stabilisation of a patient’s condition • Improved times of self-management & education programmes to ensure more patients have access to these • Access to specialist advice to primary care • Improved quality of life for service users • Enhanced ability to self-care in service users • Improved knowledge and lifestyle changes for patients • Facilitating every contact point with patients as an opportunity for education
