Home from Hospital Service for North Lincolnshire Clinical Commissioning Group

North Lincolnshire CCG wishes to commission a new service that is aimed particularly at people who do not meet the Fair Access to Care criteria for social care, or do not have family or friends to support them. The service will: • Deliver a range of support services, appropriate to individual needs, offered to those vulnerable people who without support have an above average risk of losing their independence or accommodation, but who with support stand a good chance of achieving or maintaining their independence or accommodation.” • Support vulnerable people whether they have accommodation or are homeless. • Work as part of a multi-agency approach to deliver the best outcomes for vulnerable people, establishing working relationships with North Lincolnshire CCG and North Lincolnshire Council. Services will be provided to people who are about to be discharged from hospital at wherever they consider to be their home. The provider will make the service available to individuals for up to 6 weeks, but for a majority of people, the support period will be significantly less. The service will be available seven days per week, with services delivered within the timescales required by the individual. The provider should be responsive to individual needs and provider service across a broad range of hours to reflect the times patients are discharged from hospital. Examples of the type of support required include collecting prescriptions, shopping, ensuring heating is on and working, helping sort bills and paperwork, supporting arrangements for temporary pet care if required.
