Tender description

Market testing for the provision of GP Enhanced Services for Care Homes with Nursing in Eastern Cheshire. Start date 01/10/2014- 30/09/2017 NHS Eastern Cheshire CCG includes 23 GP Practices across Eastern Cheshire with a population of 201,000, 1 District General Hospital (Macclesfield), 2 Community Hospitals (Congleton War Memorial and Knustford Community)and 1 Independent Hospital. Currently in Eastern Cheshire there are 32 Care Homes with Nursing. A number of GP Practices provide enhanced care to the specification requirements. The aim of this service is to provide a consistent level of enhanced primary care service for individuals (permanent or temporary) in Eastern Cheshire care homes with nursing. This service excludes individuals in residential care.
