Extra Care Housing Support Services

Tender description

The Council is commissioning an Extra Care Service. Extra care is an important part of our offer to older people in the borough; enabling them to live as independently as possible in the community, with care and support provided on site and tailored to individual’s needs and preferences. We want our extra care schemes to be a part of an engaging and lively community in Barking and Dagenham, with activities that are directed by, and co-produced with the residents who live there and where schemes work closely with facilities and organisations in the Borough to ensure that schemes have robust connections to their locality. The service will place a strong emphasis on the wellbeing and wellness of the residents, this will include both those receiving care and support and those who are not yet eligible. Extra care should have prevention at its heart, and schemes should focus on working with residents to prevent, reduce and delay care needs in an innovative way. The provider will harness opportunities provided by technology, information and advice and through an inclusive wellness programme which includes a continuous, proactive programme of activities which meets the diverse needs of the residents. The contract will be awarded for a period of 3 years commencing 1st February 2018 with an option to extend for a further 2 years on an annual basis at the discretion of the Council.
