Community Family Support Service

Tender description

Hull Clinical Commissioning Group (CCG) is looking for suitable providers for their community family support service. This service will provide support through home visiting volunteers, to vulnerable families who have a child under the age of 5 years, where special educational needs, disability, illness, emotional health or social isolation is evident in a member of the family. The overall aim of the Community Family Support Service is to provide additional family support to 100 families with at least one child under the age of 5 years, who are registered with a Hull GP. This community family support service will work collaboratively with statutory and other voluntary children and families services to support the delivery of local strategic objectives of the Hull Health and Wellbeing Board ‘Best Start in Life’ and the ‘Hull Early Help and Intervention Framework’ (attached).
