Tender description

Dudley Primary Care Trust (PCT) acting on behalf of Dudley Clinical Commissioning Group (CCG) are re-designing their 'falls service' to become more integrated. This will be achieved by having a central triage point for all referrals and for clinical assessments, to be based out in the community alongside the 'community falls service' based at Brierley Hill Health & Social Care Centre, Venture Way, Brierley Hill, West Midlands, DY5 1RU. To deliver on the above, we will require: 0.1 WTE ortho-geriatrician with experience of assessing falls due to syncope, blackouts, vestibular disorders, gait problems and unexplained loss of consciousness. 0.2 WTE Band 7 (Agenda for Change) nurse with experience of triage and assessments for falls. The initial award will be for two years, with an option to extend for up to a further twenty four months. Completion of the Pre Qualification Questionnaire (PQQ) forms the first stage of this process and will be used to short list organisations, and only those short listed will be taken through to the Invitation to Tender (ITT) stage. It is envisaged that a maximum of 5 organisations will be taken through to the ITT stage and this will be based on the highest scoring organisations who meet the minimum requirements. To be considered for the next stage of this process, the PQQ must be completed correctly, and submitted in ENGLISH before the closing date via our e-tendering portal BRAVO. Dudley PCT acting on behalf of Dudley CCG reserve the right to request site visits and presentations by potential suppliers as part of this process, if so required. Dudley PCT acting on behalf of Dudley CCG also reserve the right to annul the tender process, not to accept the lowest or any offer, and shall not be liable to re-imburse a tenderer for costs incurred in, or related to, the preparation of the PQQ or tender.
