NHS Nene and NHS Corby Clinical Commissioning Groups require high quality supported living Providers for individuals with learning disabilities living in Northamptonshire and who meet the Continuing Healthcare criteria. The contracts will be implemented from 1 April 2015. Please note that under the Any Qualified Provider there will be no guarantee of volume in the contracts awarded and pricing (hourly rates) is set. We are seeking application forms from Providers that can meet the service specification for the Supported Living Service (including skilled enhanced) for Adults with Learning Disability. Providers are able to submit application forms for the Standard provision and/or Skilled Enhanced provision. The aim is to provide a high quality, person centred supported living service to enable an individual to live in their own home and participate in their local community. The Provider will meet the needs, outcomes and aspirations of Adults with learning disabilities who may also have profound and multiple learning disabilities, mental illness, Autism and/or complex behaviour that includes severe behaviour that challenges others. A majority of individuals will have significant communication difficulties including a level of non-verbal communication. Individuals will generally need a high degree of health interventions and support. Providers should read the Qualification Information document and all other supporting documents, available for download on Bravo, prior to filling out the Bravo application form, paying attention to the guidance therein. The Bravo Messaging system for questions from Applicants will close on Tuesday 09 December 2014 at 12 noon, allowing time for answers to be posted and applicants to complete their applications by the reply by date of Tuesday 16 December 2014 at 12 noon. If Applicants are asked Clarification Questions during the Evaluation Stages, they have 48 hours in which to respond.
