AQP NHS GOS18 Referral Refinement Scheme for x4 Lincolnshire CCGs

This is an AQP advert published on behalf of the 4 Lincolnshire CCGs as specified CCGs. This is a repeat window of opportunity that allows additional service providers to join the existing Any Qualified Provider list (the AQP service providers’ list has been running in the 4 Lincolnshire CCGs since 2014) and provide NHS NHS GOS18 Referral Refinement Scheme. Please Note: The providers who are already on the existing AQP list do not need to re-apply. The Commissioners welcome applications for service provision in the four CCG areas and in particular to cover Gainsborough, Bourne and Long Sutton. The Commissioners require an excellent high quality patient centred Community Optometry referral refinement service which raises the standard of care, which is easily accessible and has well-designed pathways for all patients over the age of 16. The service will be available to those patients who are registered permanently or temporarily with a GP within any of the Commissioners party to the contract. In securing services under the AQP process, assurances of competence, quality, financial stability, and safety standards are required. Any provider will be eligible to join the Qualified Provider list as long as they can demonstrate competency against the specified criteria. As a Qualified Provider there are no guaranteed levels of activity and payment is made based on actual provision. All providers who wish to apply must do so by completing the Qualification Questionnaire and Technical Questionnaire which can be accessed through BravoSolution.
