Tender description

NHS North Derbyshire Clinical Commissioning Group (NDCCG) and NHS Hardwick Clinical Commissioning Group (HCCG) are commissioning a community based OHT and suspect COAG monitoring service. The number of patients who are referred for suspect Open Angle Glaucoma and then found to have no Glaucoma is around 40%. These false positive referrals cause unnecessary anxiety to the patient and are a waste of hospital resources. Level 1 - The service will be available to patients who are suspected of having glaucoma at their GOS or private eye test, for a diagnostic test to help ensure the appropriateness of referrals to hospital eye services. The service will be provided in the community by optometrists with the appropriate skills, training and equipment. Level 2 - Patients who have a confirmed diagnosis of OHT or suspect COAG and who are stable at the Hospital Eye Service i.e. are at low risk of conversion to COAG. The hospital eye service will determine the referral criteria of stable glaucoma patients from the acute trust in to the community. Please note that under ‘Any Qualified Provider’ contracts there will be no guarantees of volume in the contracts awarded. Furthermore, prices are set. Potential Providers are asked to please note the following: Providers should read the Qualification Information document and all other supporting documents, available for download on Bravo, prior to filling out the Bravo application form, paying attention to the guidance therein. The Bravo Messaging system for questions from Applicants will close on Friday 09 January 2015 at 12 noon, allowing time for answers to be posted and applicants to complete their applications by the reply by date of Friday 16 January 2015 at 12 noon. If Applicants are asked Clarification Questions during the Evaluation Stages, they have 48 hours in which to respond. Please ensure that you have staff available to answer the questions, which will be sent to you by email.
