AQP Home Care Services for Children & Young People–Nene, Corby and Milton Keynes

NHS Nene, Corby and Milton Keynes Clinical Commissioning Groups (CCGs) are seeking to establish a list of Approved Providers who can provide packages of care in the community to children and young people who meet the threshold for continuing care (CC) support in accordance with the specification. The purpose of the care package is to support parents/carers to care for their child at home within the context of their family whilst living as independently as possible. Please refer to the Service Specification, for detail of the domiciliary care provision required in this procurement. This procurement main aims are: • To establish a list of Approved Providers, who can deliver high quality, outcome focussed packages of care for children and young people living at home with their families.. • To develop a service which can meet the needs of children, young people and their families in Northamptonshire and Milton Keynes in accordance with the principles of the National Framework for Children and Young People’s Continuing Care guidance and the national personalisation agenda. Please note that under ‘Any Qualified Provider’ contracts there will be no guarantees of volume in the contracts awarded. Further, prices (e.g. hourly rates) are set. The Service Commencement date is 17 November 2014. Providers should read the Qualification Information document and all other supporting documents, available for download on Bravo, prior to filling out the Bravo application form, paying attention to the guidance therein. The Bravo Messaging system for questions from Applicants will close on Tuesday 16 September 2014 at 12 noon, allowing time for answers to be posted and applicants to complete their applications by the reply by date of Tuesday 23 September 2014 at 12 noon. If Applicants are asked Clarification Questions during the Evaluation Stages, they have 48 hours in which to respond.
