AQP for Continuing Health Care Related Services Phase 6 East Midlands CCGs

This is an AQP (Any Qualified Provider) advert published on behalf of East Midland based CCGs. This is a repeat window of opportunity that allows new service providers to join the existing Any Qualified Provider list (AQP service providers’ list which has been running in the East Midlands since Jan 2011) and provide NHS Continuing Health Care (CHC) related services. The providers who are already on the existing AQP list do not need to re-apply. The AQP process focuses on new, fully-funded packages of care for patients. This includes patients with Physical Disabilities, Learning Disabilities, Mental Health (including Dementia), Brain Injury and End of Life Care, for specified age groups 'under 65' & '65 and over'. The CCG Commissioners would like to encourage providers to apply for all CCGs across the East Midlands, as the CCGs do cross border placements of patients. In securing services under the AQP process, assurance of competence, quality and safety standards is required. Any provider will be eligible to join the Qualified Provider list as long as they can demonstrate competency against specified criteria. As a Qualified Provider there are no guaranteed levels of activity and payment is made based on actual provision. Providers should read the Qualification Information document and all other supporting documents, available for download on Bravo, prior to filling out the Bravo application form, paying attention to the guidance therein. The Bravo Messaging system for questions from Applicants will close on Friday 25 July 2014 at 12 noon, allowing time for answers to be posted and applicants to complete their applications by the reply by date of Friday 01 August 2014 at 12 noon.
