This is an AQP (Any Qualified Provider) advert published on behalf of East Midland CCGs. This is a repeat window of opportunity that allows new service providers to join the existing Any Qualified Provider list (AQP service providers’ list which has been running in the East Midlands since Jan 2011) and provide NHS Continuing Health Care (CHC) services. Please Note: The providers who are already on the existing AQP list do not need to re-apply. The AQP process focuses on new, fully-funded packages of care for patients. This includes patients with Physical Disabilities, Learning Disabilities, Mental Health (including Dementia), Brain Injury and End of Life Care, for specified age groups 'under 65' & '65 and over'. The CCG Commissioners would like to encourage providers to apply for all CCGs across the East Midlands, as the CCGs do cross border placements of patients. In securing services under the AQP process, assurance of competence, quality and safety standards is required. Any provider will be eligible to join the Qualified Provider list as long as they can demonstrate competency against specified criteria. As a Qualified Provider there are no guaranteed levels of activity and payment is made based on actual provision. The Bravo Messaging system for questions from Applicants will close on Tuesday 21 April 2015 at 12 noon, allowing time for answers to be posted and applicants to complete their applications by the reply by date of Tuesday 28 April 2015 at 12 noon. Please note that we reserve the right to review and amend all dates given within this notice and within this procurement process. If Applicants are asked Clarification Questions during the Evaluation Stages, they have 48 hours in which to respond. Please ensure that you have staff available to answer the questions, which will be sent to you by email to the email address registered on Bravo. Failure to respond in the given time could adversely impact your application.
