AQP Domiciliary Care Nene and Corby CCGs

Tender description

NHS Nene and NHS Corby Clinical Commissioning Groups require high quality domiciliary care CHC services for adults, aged 18 or over, in Northamptonshire. We plan to implement these new contracts from September 2014. Please note that under ‘Any Qualified Provider’ contracts there will be no guarantees of volume in the contracts awarded. Further, prices (e.g. hourly rates) are set. We are seeking expressions of interest from: • Interim contract holders seeking to become Approved providers • New providers wishing to provide CHC domiciliary care services We welcome expressions of interest from providers who provide high quality, personalised care to individuals in their own home (including 24 hour support and rehabilitation services) or as an addendum to an existing residential based setting. Supporting people and their carers who have additional health needs arising from frailty, illness, accident or disability to achieve the best possible quality of life and wellbeing. Services include providing care to the following care groups: Learning disability, physical disability, older people mental health, neurological conditions, brain injury, end of life care, palliative care Services are provided in the following geographical zones: • Urban areas: Corby, Kettering, Northampton, Rushden, Thrapston, Daventry and Wellingborough • Rural areas: East Northants, South Northants, Brackley and Towcester Providers should read the Qualification Information document and all other supporting documents, available for download on Bravo, prior to filling out the Bravo application form, paying attention to the guidance therein. The Bravo Messaging system for questions from Applicants will close on Wednesday 25 June 2014 at 12 noon, allowing time for answers to be posted and applicants to complete their applications by the reply by date of Tuesday 08 July 2014 at 12 noon.
