Diabetes services for North East Essex are currently delivered through a traditional medical model with a range of services commissioned from secondary, primary and community care providers. Although there have been improvements in diabetes care in the last two years, incremental improvements have not resulted in the desired level of improvement in patient outcomes or pathway efficiencies; mortality rates remain high in comparison to similar PCT’s, outpatient attendances are significantly higher and DSR for diabetic lower limb amputations are higher than national, regional and rates in similar PCT’s. Against this background it is necessary to effect a step change, which could be achieved through the commissioning of an Integrated Pathway Hub (IPH). The key objective of the Procurement is to secure provision of an Integrated Pathway Hub that successfully delivers: High quality care, respect and dignity for all people with diabetes The overall aim of the Integrated Pathway Hub is to co-ordinate, promote, and ensure via a single point of access the provision of a comprehensive range of high quality, cost effective integrated health Services for people with diabetes and their carers. To promote the concept of the service and also to provide an opportunity for potential providers to meet, we are holding an open Bidder Event during the afternoon of Wednesday 27 March 13. Please advise your interest in attending the event using the messaging function within the portal and no later than 12 noon on Friday 22 March 13. The PQQ documentation will be available for download within the Diabetes IPH PQQ area (pqq 576) on the Bravo procurement portal after 18 March 2013. Completed PQQ responses must be submitted via the procurement portal no later than 12 noon on Tuesday 7 May 13.
