<p style="margin-left: 36pt">Using the flexibilities of the Better Care Fund, Frimley ICS and Bracknell Forest Council are looking to commission a contract to deliver a Home from Hospital service. This contract will cover eligible patients within Bracknell Forest who are being discharged from hospital. The service will be managed by a coordinator who will liaise with key stakeholders to receive and action referrals.</p> <p style="margin-left: 36pt">Patients will be supported for a period of six weeks, with visits and telephone calls to help with emotional needs and small practical tasks. The service is not expected to provide personal care.
