Tender description

<p>The aim of this Test &amp; Learn project is to support social care discharges from A&amp;E by providing a temporary placement in an OFSTED registered and regulated children’s home for children and young people (YP) that have been assessed as medically fit for discharge by the acute hospital. However, they are unable to return to their home or their LAC placement immediately, due to their needs, associated risks or crisis at home. The temporary placement is for a maximum of eight weeks and the intention is for the young people placed to return to their home or previous placement wherever possible and in the shortest appropriate timescale. Central &amp; North West London NHS Trust (CNWL) and West London NHS Trust (WL) have funded the Test &amp; Learn project. Brent Local Authority (LA) will be the lead commissioner. The maximum budget is £483,000.00 fully inclusive of any VAT payable by the Council. Funding for the initial period is expected to be for 6-9 months, which may or may not be extended subject to a review of the pilot.</p> Keywords: Hospital, Discharge, Children, Care
