Tender description

The Authority is seeking to commission up to three care providers who will have sole responsibility for the delivery of care and support at each of the Extra Care facilities that is focussed on delivering outcomes for the people living in Hull Extra Care. This model will ensure that a person centred approach is undertaken at all times using a strengths based approach. The contracts will be for 5 years with the option to extend for a further 2 periods of 12 months each.. Lot 46715: Lot 1 - Cecil Gardens The Authority is seeking to commission up to three care providers who will have primary responsibility for the delivery of care and support at each of the extra care facilities that is focussed on delivering outcomes for the people living in Hull Extra Care. This model will ensure that a person-centred approach is undertaken at all times using a strengths based approach. The contracts will be for 5 years with the option to extend for a further 2 periods of 12 months each.Please ensure you complete the Lot preference table located within ITT Part 2 Lot 47838: Lot 2 - Harrison Park The Authority is seeking to commission up to three care providers who will have primary responsibility for the delivery of care and support at each of the extra care facilities that is focussed on delivering outcomes for the people living in Hull Extra Care. This model will ensure that a person-centred approach is undertaken at all times using a strengths based approach. The contracts will be for 5 years with the option to extend for a further 2 periods of 12 months each. Please ensure you complete the Lot preference table located within ITT Part 2 Lot 47839: Lot 3 - Redwood Glades The Authority is seeking to commission up to three care providers who will have primary responsibility for the delivery of care and support at each of the extra care facilities that is focussed on delivering outcomes for the people living in Hull Extra Care. This model will ensure that a person-centred approach is undertaken at all times using a strengths based approach. The contracts will be for 5 years with the option to extend for a further 2 periods of 12 months each. Please ensure you complete the Lot preference table which is located within ITT Part 2.
