Tender description
The aim of this service is to be a recovery focused service that supports adults with mental health needs to maintain their independence through meaningful interventions and defined outcomes. The service shall deliver upon this aim by providing: • An assertive outreach approach that is flexible and builds relationships with people who have difficulties engaging with services. • Support to people to engage in community activities and attend appointments, using a graded approach with the aim of independence. • Support to people to manage their mental health symptoms and challenging behaviour. • Support with tenancy sustainment which may include repairs, bills, benefits and other correspondence where appropriate. • Utilising the recovery star, or equivalent, methodology to track progress on an ongoing basis with the service users. Lot 1: High: Complex and forensic needs. Staff will be on-site 24/7. The aim of this service is to be a recovery focused service that supports adults with mental health needs to maintain their independence through meaningful interventions and defined outcomes. The service shall deliver upon this aim by providing: • An assertive outreach approach that is flexible and builds relationships with people who have difficulties engaging with services. • Support to people to engage in community activities and attend appointments, using a graded approach with the aim of independence. • Support to people to manage their mental health symptoms and challenging behaviour. • Support with tenancy sustainment which may include repairs, bills, benefits and other correspondence where appropriate. • Utilising the recovery star, or equivalent, methodology to track progress on an ongoing basis with the service users. Lot 2: Medium: Service users will receive up to 9 hours of support a week, and there will be one waking night per scheme and either concierge or security The aim of this service is to be a recovery focused service that supports adults with mental health needs to maintain their independence through meaningful interventions and defined outcomes. The service shall deliver upon this aim by providing: • An assertive outreach approach that is flexible and builds relationships with people who have difficulties engaging with services. • Support to people to engage in community activities and attend appointments, using a graded approach with the aim of independence. • Support to people to manage their mental health symptoms and challenging behaviour. • Support with tenancy sustainment which may include repairs, bills, benefits and other correspondence where appropriate. • Utilising the recovery star, or equivalent, methodology to track progress on an ongoing basis with the service users. Lot 3: Medium/Low: Providing support up to 9 hours per week. Out of hours support at 4 hours per night with concierge or visiting security. The aim of this service is to be a recovery focused service that supports adults with mental health needs to maintain their independence through meaningful interventions and defined outcomes. The service shall deliver upon this aim by providing: • An assertive outreach approach that is flexible and builds relationships with people who have difficulties engaging with services. • Support to people to engage in community activities and attend appointments, using a graded approach with the aim of independence. • Support to people to manage their mental health symptoms and challenging behaviour. • Support with tenancy sustainment which may include repairs, bills, benefits and other correspondence where appropriate. • Utilising the recovery star, or equivalent, methodology to track progress on an ongoing basis with the service users. Lot 4: Medium/Low: 1:1 support available service users will receive up to 4 hours of support a week The aim of this service is to be a recovery focused service that supports adults with mental health needs to maintain their independence through meaningful interventions and defined outcomes. The service shall deliver upon this aim by providing: • An assertive outreach approach that is flexible and builds relationships with people who have difficulties engaging with services. • Support to people to engage in community activities and attend appointments, using a graded approach with the aim of independence. • Support to people to manage their mental health symptoms and challenging behaviour. • Support with tenancy sustainment which may include repairs, bills, benefits and other correspondence where appropriate. • Utilising the recovery star, or equivalent, methodology to track progress on an ongoing basis with the service users. Lot 5: Low: Provide flexible support up to 4 hours per week to meet individual support needs. The aim of this service is to be a recovery focused service that supports adults with mental health needs to maintain their independence through meaningful interventions and defined outcomes. The service shall deliver upon this aim by providing: • An assertive outreach approach that is flexible and builds relationships with people who have difficulties engaging with services. • Support to people to engage in community activities and attend appointments, using a graded approach with the aim of independence. • Support to people to manage their mental health symptoms and challenging behaviour. • Support with tenancy sustainment which may include repairs, bills, benefits and other correspondence where appropriate. • Utilising the recovery star, or equivalent, methodology to track progress on an ongoing basis with the service users.
