1.1 Quotes are sought from a suitably qualified and experienced Provider to provide Independent Facilitation for a series of North Powys Wellbeing Programme Transformation Workshops. The half-day workshops will total 15 in all, with five Transformation areas having three phases. 1.1.1 The Transformation areas are as follows: a) Children and Young People b) Social Model for Health c) Integrated Community Model and Frailty d) Mental Health, Learning Disabilities and Wellbeing. e) Diagnostics, Ambulatory and Planned Care
