<p>Somerset NHS Foundation Trust is embarking upon an exciting project to develop community respiratory and frailty services under the auspices of the NHS Hospital@Home initiative, the aim being to reduce the dependence of patients and other agencies upon acute sector beds.&nbsp;Other specialities may also be added to the scope of works during the contract period.</p> <p>The Somerset Virtual Ward will commence shortly. The team will provide support for both early hospital discharge and admissions avoidance, undertake a variety of interventions to support patients, including home assessment, telephone/video contact and the application of digital monitoring systems to manage a caseload. The team will link seamlessly with colleagues in primary care, secondary care, and other developing elements of the Hospital@Home service, so providing virtual ward support for patients who would otherwise be in hospital. The project is being implemented in Test and Learn Phases initially starting with a small patient group, but numbers will increase to approx. 300 over time. It is expected that on successful award of contract that a pilot be in place for early 2023 with a view to full roll out of the system by mid-2023. It is envisaged that following on from a successful pilot the system will become a staple of the hospital systems until at least 2025. Contract Value Approx. £180,000 The trust welcomes expressions of interest from suppliers who can meet the set requirements and be available to attend either in person (preferred) or virtually at the trust on the 7th of October to meet the clinicians and demonstrate the system. These demonstrations will help the Trust to develop a specification for a future procurement exercise.
