City & Hackney Enhanced Health Visiting Service

<p>To provide a holistic Enhanced Health Visiting Service which provides expert advice, support and interventions to families with children in the first years of life, in line with the revised 2021 national Healthy Child Programme (HCP). The service must be ‘universal in reach and personalised in response’ and work with families with children aged 0 to 5yrs to identify health needs as early as possible, improve access to services and improve health and wellbeing by promoting health, preventing ill health and reducing inequalities in the populations of the London Borough of Hackney and the City of London.</p> <p><span style="font-weight: 400">Service provision is to be provided at five levels:</span></p> <p><span style="font-weight: 400">Level 1: Community&nbsp;</span></p> <p><span style="font-weight: 400">Level 2: Universal - Mandated Reviews</span></p> <ul> <li style="font-weight: 400"><span style="font-weight: 400">Antenatal&nbsp;</span></li> <li style="font-weight: 400"><span style="font-weight: 400">New Baby&nbsp;&nbsp;</span></li> <li style="font-weight: 400"><span style="font-weight: 400">6-8 weeks&nbsp;&nbsp;</span></li> <li style="font-weight: 400"><span style="font-weight: 400">1 year</span></li> <li style="font-weight: 400"><span style="font-weight: 400">a 18 month- desktop review of all child health records</span></li> <li style="font-weight: 400"><span style="font-weight: 400">2-2 ½ year</span></li> </ul> <p><span style="font-weight: 400">Level 3: Targeted</span></p> <ul> <li style="font-weight: 400"><span style="font-weight: 400">One month&nbsp;</span></li> <li style="font-weight: 400"><span style="font-weight: 400">3-4 months&nbsp;</span></li> <li style="font-weight: 400"><span style="font-weight: 400">3 - 3 1/2 years</span></li> </ul> <p><span style="font-weight: 400">Level 4: Specialist</span></p> <ul> <li style="font-weight: 400"><span style="font-weight: 400">Multi-agency working</span></li> </ul> <p><span style="font-weight: 400">Level 5: Intensive&nbsp;</span></p> <ul> <li style="font-weight: 400"><span style="font-weight: 400">Home visiting service for vulnerable families with children under 2yrs, open to families regardless of age, number of children and not restricted to first pregnancies</span></li> </ul> <p><span style="font-weight: 400">As the lead for the Healthy Child Programme 0- 5yrs, </span><span style="font-weight: 400">the Enhanced Health Visiting service must also have 9 High Impact roles in line with the needs of the City &amp; Hackney population:</span></p> <ol> <li style="font-weight: 400"><span style="font-weight: 400">Breastfeeding</span></li> <li style="font-weight: 400"><span style="font-weight: 400">Transition to Parenthood</span></li> <li style="font-weight: 400"><span style="font-weight: 400">Perinatal Mental Health</span></li> <li style="font-weight: 400"><span style="font-weight: 400">Special additional needs&nbsp;</span></li> <li style="font-weight: 400"><span style="font-weight: 400">Healthy Weight</span></li> <li style="font-weight: 400"><span style="font-weight: 400">Substance Misuse</span></li> <li style="font-weight: 400"><span style="font-weight: 400">A&amp;E Liaison Nurse</span></li> <li style="font-weight: 400"><span style="font-weight: 400">Lead for Integrated Reviews</span></li> <li style="font-weight: 400"><span style="font-weight: 400">Homelessness</span><span style="font-weight: 400">&nbsp;&nbsp;</span></li> </ol> <p>&nbsp;</p> <p><span style="font-weight: 400">The anticipated total value of this contact will be a maximum of £32,091,624.60 over 5 years.&nbsp;</span></p> <p><span style="font-weight: 400">If you would like to register your interest in this opportunity, please contact the London Borough of Hackney.
