FRAMEWORK FOR THE PROVISION OF HOME-BASED SUPPORT SERVICES WITHIN THE BOROUGH OF WALTHAM FOREST

The London Borough of Waltham Forest Under the Care Act 2014 the Council has the statutory obligation to support individuals who, following assessment, are deemed to have eligible needs requiring care and support. The new Framework model aims to strengthen strategic relationships between Adult Social Care and home-based support providers, ensuring that quality conversations can be had between home-based support workers, care management and therapeutic professionals. The Council of the London Borough of Waltham Forest is seeking Tenders from suitably experienced and qualified organisations for the Framework Agreement for Provision of Home Based Support service for individuals with eligible needs requiring support in the London Borough of Waltham Forest The main purpose of the Service required under the Framework is to: • Safeguard those individuals who need the most support/ who are most at risk • Promote self-reliance and personal responsibility • Build resilience and competence for individuals, families and carers • Enable better care experiences and outcomes • Strike a balance between empowerment and safeguarding • Strengthen relationships with the Authority’s partners, to deliver improved outcomes and to deliver greater productivity.
