Tender description

<p>NHS Northamptonshire Clinical Commissioning Group is procuring a provider for the Earls Barton Medical Centre and Penvale Park Surgery Service on an APMS.</p> <p>It is expected that the contract will be for a proposed period of 10 years with the opportunity to extend for a further period of up to 5 years at the discretion of the Contracting Authority (total 15 years). The estimated annual contract value is approximately &pound;792,445.00 per annum.</p> <p>The Commissioner is seeking the provision of integrated primary care services, which align to both the national and local strategic plan including, but not limited to:</p> <ul> <li>Develop new ways to improve access and service delivery for our diverse population (where relevant, learning from the pandemic to embed innovative ways to deliver services that meet patient needs);</li> <li>Improve and embed the ethos of integration and collaborative working with local health and social care providers, including &lsquo;Primary Care Networks;</li> <li>Embed a culture of co-design with patients in the development of integrated primary care services ensuring the patient has more control over their own health and the care they receive.</li> <li>Understand and adopt a population health management approach to tackling health inequalities;</li> <li>Enable a service model that increases equity of both access and outcomes for our different sections of the practice population;</li> <li>Promote physical and psychological well-being and self- care in a holistic model;</li> <li>The provider must support the &lsquo;digital first&rsquo; approach and use clinically safe, NHS approved digital technology to improve access, increase equity of outcome, increase patient choice, and improve quality of care</li> <li>The provider must provide a GP Led service with appropriate clinical oversight to support the multi- disciplinary practice team;</li> <li>The provider must provide and support all staff holistically, ensuring personal and professional development, promote confidence and greater staff satisfaction.</li> </ul> <p>The procurement is being run via the ProContract (Proactis) e-system where all procurement documents are available for download.&nbsp;</p> Keywords: Northamptonshire, Earls Barton, Penvale Park, APMS
