Health and wellbeing support to rough sleepers; those newly accessing temporary/supported accommodation and those who are about to leave supported accommodation in order to facilitate access into appropriate accommodation and move on. Through individual specialisms, the various providers will also provide support to the wider team. Support to be provided by 3 Mental Health workers, 1 each from Pentreath, Rethink and Cornwall Mind
