Ukraine response: new patient health questionnaire

Tender description

Building on current new patient questionnaires available in general practice and other relevant materials , develop a new patient questionnaire which can be issued to and used by GP practices to support the initial health assessment and management of newly registered vulnerable migrants including people seeking asylum and those arriving under Government relocation schemes. This includes patients whose first language may not be English. Given the current conflict in Ukraine the priority is to establish a new dual language patient questionnaire to support new patient registrations in to general practice for people coming to the UK from Ukraine supporting assessment of their health care needs.
