Tender description

Bespoke commissioning of domiciliary care as part of a Notional Personal Health Budget (PHB) package of care to meet assessed needs of children eligible for CCC where the responsible commissioner is NHS Nottingham and Nottinghamshire CCG (Including Bassetlaw). There are between 60 and 100 children eligible for CCC at any time, most of whom require care in their home for up to 105 hours each week to remain safe and well. The number of children fluctuates according to which universal and specialist services are commissioned locally. Children’s needs are reviewed at least annually and therefore this cohort is not static. The service is: *an individually commissioned, patient centred approach within care planning and provision that facilitates the voice of the child and parent and maximizes child and parent autonomy and engagement. * is managed by appropriately qualified and registered managers, usually a Children’s nurse. Care and support will be delivered by appropriately trained registered nurses or delegated to care workers who are competent to meet the needs of the individual child. Each child will have a named registered Children’s Nurse with responsibility and accountability for identifying needs and delivering commissioned care. *one where service planning and delivery recognizes and is able to accommodate that a child has individual rights and needs which might take precedence over the wishes and feelings of parents and carers. Planning and service delivery will also recognize the child’s religion, sexuality, racial origin, culture and language * able to ensure that a child’s right to privacy is respected but care package arrangements which affect the child, parent, carer or significant other are discussed with the parties involved. assured that the child.
