Tender description
To use additional social prescribing link worker (SPLW) resource to contact all eligible patients who are recorded as 91 days since their first dose of the C-19 vaccination and who fall into the priority groups. Initially, the focus will be on those patients who are recorded as 250 days + since their first dose. This is a bespoke service set up to support the vaccination programme. It will be funded by the COMF budget
