Tender description
<p>The Northwest London CCG are seeking a Learning Disability, Autism and Mental Health specialist community care provider for a 32-year-old male with the diagnosis of autism and moderate learning disability. We are looking for a placement within London; preferably Hounslow and the surrounding areas. This community placement (supported living accommodation) is required to facilitate a transfer from an acute hospital setting because the individual is fit for discharge. The placement required is a small care setting with a maximum of three other male residents. NW London CCG would also consider independently sourcing a property but seek a community based-care provider to provide care and support.
