This contract is broken down into 2 lots: Lot 1 – The Provision of a Community Link Workers Service - Inverclyde Lot 2 – The Provision of a Community Link Workers Service - Renfrewshire Tenderers may choose to apply for one or both lots, with one supplier delivering each lot in its entirety. Lot 1: Lot 1 – The Provision of a Community Link Workers Service - Inverclyde Inverclyde Health & Social Care Partnership (IHSCP) is seeking a Provider to deliver a Community Link Worker Service to all General Practices in Inverclyde. CLW’s work predominately with adults over the age of 16 and their families in areas of deprivation, along with population groups who experience significant social and health inequalities. Lot 2: Lot 2 – The Provision of a Community Link Workers Service - Renfrewshire Renfrewshire Health & Social Care Partnership (RHSCP) is seeking a Provider to deliver a Community Link Workers Service to all General Practices in Renfrewshire.
