<p>Cheshire East Council are looking to award a contract for Early Help Visual Impairment services. The emphasis of this service will be on providing practical and emotional support, and strategies for independent living including low vision support through a holistic assessment of need using a person-centred and strength-based approach. The service requires a co-ordinated, collaborative approach to providing services for people with visual impairments. This will ensure integrated pathways of support are provided across social care, health, education, and the local voluntary and community sector commissioned services, to improve outcomes for people. This will facilitate a whole systems approach and holistic provision for people accessing the service and avoiding duplication.
